TO BOOK AN APPOINTMENT

There are several ways to request an appointment:

Email your request, preferably with a scanned copy (or picture) of your requisition. Email requests will be attended to within 24-48 hours.

By Phone: 613-443-2095

We try to return messages within 24-48 hours.

By Fax: 613-443-5625

You may request your health-care provider fax us a requisition. If your provider is faxing us a requisition, you do not need to call us, we will call you within 24 hours of receiving it.

X-RAY WALK-INS ACCEPTED

We are now accepting X-Ray Walk-Ins. NO APPOINTMENT REQUIRED! *Please view our "HOME" page for updates and closures

For X-Rays, you can come in anytime Monday through Friday from 8:00 a.m. to 3:30 p.m.

You may arrive as early as 8:00 a.m. to check-in.
Our tech will start seeing patients around 8:30 a.m.
Our tech takes a quick lunch from 12:00 p.m. to 12:30 p.m.
3:30 p.m. is LAST CALL. We strongly advise arriving earlier because we close X-Ray walk-ins if we reach capacity.

Please be aware that wait times can range anywhere from 5-90 minutes.

X-Ray walk-ins are accepted up until 30 minutes before closing (3:30 pm).

PRIOR TO BOOKING/WALKING-IN

Requisition Information

Before booking or attending a walk-in appointment, your healthcare provider must provide you with a requisition requesting an exam. Alternatively, this requistion may be sent to us via FAX or email by your healthcare provider.

Please ensure your requisition has been filled out correctly and includes the following:

YOUR (PATIENT'S) FULL INFORMATION

(IE. NAME, DATE OF BIRTH, PHONE #)

SELECTION OF REQUIRED EXAM

(IE. BMD, ULTRASOUND, OR X-RAY)

COMPLETED CLINICAL INFORMATION/CLINICAL HISTORY
YOUR HEALTHCARE PROVIDER'S: SIGNATURE, FULL NAME, ADDRESS, PHONE #, & FAX #

If you have a hard copy of your requisition, bring this along with your Ontario OHIP+ card to your appointment.
